Does a fee target guarantee confirmation?

No. Block space demand and block arrival times change. A rate estimate cannot reserve space in a block or promise a deadline.

Why might the dollar fee differ from the market ticker?

The dollar estimate uses the BTC price you enter in the form, which starts with an illustrative value. The fee in satoshis does not change when you edit only the BTC price.

Where can I find my transaction size?

Look for virtual size in your wallet's transaction preview or coin-control details. The starting 140 vB is an example and cannot describe every payment.
